There are 559 Medicare ACOs across the U.S., according to the National Association of ACOs. Around 12.3 million Medicare beneficiaries — 20 percent of all Medicare beneficiaries — participate in an ACO, and hundreds of other patients participate in commercial and Medicaid ACOs.

CMS launched the Medicare Shared Savings Program in 2012, which includes three tracks, to encourage voluntary participation in ACO programs. In 2018, the agency added Track 1+, which offers lower shared risk than existing tracks and ACO models. The Next Generation ACO model launched in 2016, requiring providers to take on greater financial risk with the potential for more shared savings.

Becker's developed this list to highlight ACOs across the country and examine the opportunities for improving quality care and care coordination. The editorial team accepted nominations for this list and conducted independent research.

Accountable Care Coalition of Southeast Wisconsin (Milwaukee). Accountable Care Coalition of Southeast Wisconsin launched in April 2012 and is on its second agreement period. In the 2017 performance year, the Track 2 MSSP ACO reported $2.2 million in shared savings and distributed 17 percent to participants. Another 66 percent was reinvested to redesign care processes and resources, and the rest went toward infrastructure.

Accountable Care Coalition of Texas (Houston). Accountable Care Coalition of Texas is a Track 1 MSSP ACO that launched in April 2012 and began its second service agreement in January 2016. The ACO has 48 participants and reported $1.5 million shared savings in the 2016 performance year.

Accountable Care Options (Boynton Beach, Fla.). Accountable Care Options participates in the Next Generation ACO model, an initiative to coordinate care for populations. The ACO began participating in the MSSP in 2013, when it reported $6.25 million shared savings for the performance year. During the 2017 performance year, the ACO reported $8.74 million in shared savings and distributed 55.5 percent to the providers.

Accountable Care Organization of Aurora (Milwaukee). Accountable Care Organization of Aurora is a Track 1 MSSP ACO that serves patients in Wisconsin and Illinois. It launched in January 2018 and is in its first agreement period. The ACO has 12 participants, including Green Bay, Wis.-based BayCare Aurora, which also participates in the joint venture.

ACO West Virginia (Morgantown). ACO West Virginia serves patients in Maryland, Pennsylvania, Ohio and West Virginia. The Track 1 MSSP ACO launched in January 2018 and is in its initial agreement period. The ACO is affiliated with Morgantown-based WVU Medicine, a 10-hospital health system that is the largest employer in West Virginia.

Adena Healthcare Collaborative (Chillicothe, Ohio). The Ohio-based Adena Healthcare Collaborative began its initial agreement period in January 2015 and entered into its second agreement period in January 2018. The Track 1 MSSP ACO has four participants, including the three-hospital Adena Health System that has 3,000 employees, and Adena Medical Group, both based in Chillicothe. The ACO reported $4.4 million in shared savings for the 2016 performance year.

Adventist Health Accountable Care (Glendale, Calif.). Adventist Health Accountable Care is a California-based ACO that launched in January 2018. The Track 1 MSSP ACO is in its initial agreement period and has 26 participants, including Glendale, Calif.-based Adventist Health Physician Network, which has 21 locations on the West Coast and Hawaii.

AdventHealth ACO (Maitland, Fla.). Launched in January 2018, AdventHealth ACO is a Track 1 MSSP ACO. The organization has about 55,000 patients and aims to provide highly coordinated care.

Advocate Physician Partners Accountable Care (Downers Grove, Ill.). Advocate Physician Partners Accountable Care launched its initial agreement period in July 2012 and is now a Track 1+ MSSP ACO. The ACO comprises more than 5,000 physicians, 12 hospitals and a post-acute network that provides care for patients in the Chicago area and central Illinois. The organization reported 27 quality measures that exceeded the 90th percentile compared to national benchmarks in 2018.

Alabama Physician Network (Birmingham). The Alabama Physician Network is a clinically integrated network formed in 2017 to participate in the MSSP ACO program as a subsidiary of UAB Health. The physician-led organization includes the university's faculty physician group, Christ Health Center and Cahaba Medical Center, both in Birmingham. The ACO covers 16,000 lives and focuses on coordinating care while avoiding medical errors.

Aledade (Bethesda, Md.). Founded in 2014, Aledade partners with independent primary care physicians to launch ACOs. It currently operates 27 ACOs, which participate in both the Medicare Shared Savings Program and commercial contracts. Aledade has grown to include more than 430 practices that serve more than 650,000 patients in 25 states.

AllCare Health Alliance (Camden, N.J.). AllCare Health Alliance is a primary care physician organization serving South Jersey. The ACO provides coordinated care for Medicare beneficiaries and aims to improve quality while lowering costs. Launched in 2017, AllCare Health Alliance is a Track 1 MSSP ACO.

Amita Health Accountable Care Organization (Arlington Heights, Ill.). Amita Health Accountable Care Organization is in its second agreement period. Launched in January 2013, the Track 1+ MSSP ACO serves Illinois patients. The organization reported $20 million in shared savings for the 2016 performance year.

AnewCare (Johnson City, Tenn.). AnewCare Collaborative is a Tennessee-based Track 1 MSSP ACO. Launched in July 2012, the ACO is in its second agreement period and serves patients in Virginia and Tennessee. During the 2016 performance year, the ACO reported $2.6 million in shared savings and distributed 35 percent to participants, which include Johnson City, Tenn.-based Dickenson Community Hospital, Abingdon (Va.) Physician Partners and the Mountain State Health Alliance in Johnson City.

Atlantic Accountable Care Organization (Morristown, N.J.). Atlantic Accountable Care Organization launched in April 2012, and the Track 1 MSSP ACO is now in its second agreement period. The ACO has about 2,370 participants and covers about 237,000 lives. Through 2017, the ACO paid more than $30 million to provider participants and achieved a quality rating of more than 90 percent for five straight years. In 2017, it entered into a downside commercial risk contract for 28,000 beneficiaries of Medicare Advantage plans.

Atrius Health (Newton, Mass.). Atrius Health is a nonprofit health system that cares for more than 745,000 patients in eastern Massachusetts. It has 715 physicians and primary care providers and 31 practice locations. The system's ACO participates in the Next Generation ACO Model to continue taking on risk. During the 2017 performance year, Atrius Health ACO produced $19.9 million in savings and received $15 million.

Arizona Care Network (Phoenix). San Francisco-based Dignity Health founded Arizona Care Network in 2012 and entered into the MSSP in 2013. Abrazo Community Health Network partnered with Dignity to govern the ACO, which is also affiliated with Phoenix Children's Care Network. The ACO was among the first 45 organizations to participate in the Next Generation ACO in 2017 and now has 13 value-based arrangements with both commercial and Medicare payers. The organization manages care for about 300,000 patients and has 5,500 providers.

Ascension Care Management (Nashville, Tenn.). Launched in July 2012, Ascension Care Management Health Partners Tennessee is a Track 3 MSSP ACO. Ascension Care Management Health Partners has two other Track 1 MSSP ACOs, one based in Indianapolis and the other in Evansville, Ill. The organization is affiliated with St. Louis-based Ascension, a faith-based health system with about 156,000 associates and 34,000 aligned providers in 21 states.

Aurora Accountable Care Organization (Milwaukee). Aurora Accountable Care Organization launched in January 2017 and is now a Track 3 MSSP ACO. The ACO is affiliated with Milwaukee-based Aurora Health Care, a 15-hospital integrated health system that serves about 1.2 million patients per year. The system merged with Downers Grove, Ill.-based Advocate Health Care in 2018.

Banner Health Network (Mesa, Ariz.). Banner Health Network is an Arizona-based Track 3 MSSP ACO that launched in January 2017. It joins Phoenix-based Banner Health with a network of primary and specialty physicians that provide care for patients in Maricopa and Pinal counties in Arizona. The ACO includes more than 5,000 physicians and advanced care practitioners and covers more than 400,000 members and beneficiaries. Thirteen Banner Health hospitals and the MD Anderson Cancer Center in Houston participate in the organization.

Banner Network Colorado (Greeley). Banner Care Network includes 300 primary and specialty providers that care for patients in northern Colorado. About 60 percent of the providers are affiliated with Banner Health, a nonprofit health system based in Phoenix with more than 50,000 employees across its locations in Arizona and Colorado. The organization includes three hospitals and two surgery centers.

Baptist Physician Partners (Jacksonville, Fla.). Baptist Physician Partners launched in January 2018 and serves patients in Florida and Georgia. The Track 1 MSSP ACO is still in its first agreement period. The network board includes 11 physicians and five South Miami, Fla.-based Baptist Health executives who oversee the organization's efforts to improve quality and reduce costs. The ACO had five value-based contracts and had cared for more than 62,000 lives as of March 2018.

Baycare Health Partners (Springfield, Mass.). As a physician-hospital ACO, Baycare Health Partners serves four Springfield, Mass.-based Baystate Health hospitals and more than 200 medical practices. The network includes about 1,300 physicians and works with managed care organizations as well as government payers.

BayCare Physician Partners (Clearwater, Fla.). BayCare Physician Partners began as a Track 1 MSSP in 2015 and generated savings for three years before moving on to Track 3 in 2018. The ACO reported three years of savings above $30 million and quality scores consistently hitting 97 percent or more. The ACO includes 100 participants and covers around 46,000 lives.

Baylor Scott & White Quality Alliance (Dallas). Baylor Scott & White Quality Alliance is a Track 1 MSSP ACO that launched in 2015 and entered into its second agreement period in January 2018. The ACO includes 16 hospitals and two provider networks as well as other health centers and clinics. The organization is affiliated with Dallas-based Baylor Scott & White, a 50-hospital health system that has more than 7,500 physicians and a health plan.

Beacon Health (Brewer, Maine). Beacon Rural Health Partners in Brewer, Maine, launched its ACO in January 2016 as a Track 1 MSSP ACO. Beacon Health grew from 9,000 patients to 100,000 patients, including 18,000 Beacon Health employees and dependents that use the Northern Light Employee Health Plan. The organization has more than 50 providers and achieved accreditation through the National Committee for Quality Assurance.

Beaumont Accountable Care Organization (Southfield, Mich.). Beaumont Accountable Care Organization launched in 2012. In July, it entered into a new MSSP contract with CMS, selecting the enhanced track. The new contract increased share rates from 50 percent to 75 percent of savings. Since inception, the ACO has generated more than $80 million in savings for CMS and received about $40 million in shared savings. It now has more than 500 physician participants and 25,000 covered lives.

Bellin Health Partners (Green Bay, Wis.). Bellin Health in Green Bay is a clinically integrated network that includes the 220-bed Bellin Hospital in Green Bay and 86 independent specialty physicians. The system's 147-member primary and specialty care organization also participates in the ACO. During the second year of its participation in the Next Generation ACO Model, the organization reported nearly $5 million in shared savings and reinvested all in infrastructure to support quality care.

Beth Israel Deaconess Care Organization (Westwood, Mass.). The ACO includes 2,500 primary care physicians and specialists who provide care at several hospitals in the Boston area, including New England Baptist Hospital, three Beth Israel Deaconess Hospital locations and Cambridge (Mass.) Health Alliance. Beth Israel Deaconess Care Organization is also affiliated with Boston-based Harvard Medical School Teaching Hospital and provides several population health tools to its provider community.

Bethesda Health Quality Alliance (Boynton Beach, Fla.). Launched in January 2018, the Bethesda Health Quality Alliance is a Track 1 MSSP ACO in its first agreement period. The clinically integrated network is physician-led.

Billings (Mont.) Clinic. Launched in January 2013, Billings Clinic's ACO is a Track 1 MSSP organization. It began its second agreement period in January 2016 and serves patients in Montana and Wyoming. The ACO is affiliated with Billings (Mont.) Clinic, a physician-led integrated multispecialty group practice with a 304-bed hospital and level 2 trauma center. The clinic has about 4,000 staff members and 400 physicians.

BJC HealthCare ACO (St. Louis). BJC HealthCare ACO is a Track 3 MSSP ACO that launched in July 2012. It is now in its second agreement period and serves patients in Missouri and Illinois. During the 2017 performance year, BJC HealthCare ACO reported nearly $9 million in shared savings and distributed 67 percent of the savings to participants. The ACO is affiliated with St. Louis-based BJC HealthCare and includes 13 of the system's hospitals.

Boston Accountable Care Organization. Boston Accountable Care Organization includes primary care physicians from several Boston area groups, including Boston University Affiliated Physicians. It is a managed care organization under MassHealth, which provides administrative functions for managing the network. It is affiliated with Boston Medical Center Health System and aims to innovate in high quality, low cost care. The ACO has 15 participants and covers about 110,000 lives. It also launched a complex care management program that tripled assessment completion rate within 72 hours and reduced inpatient admissions by 29 percent during the three-to-six-month period after enrollment.

Broward Guardian (Hollywood, Fla.). Broward Guardian is a primary care physician organization aligned with Hollywood, Fla.-based Memorial Health System. It is a Track 3 MSSP ACO that launched in January 2014. It began its second reporting period in January 2018. During the 2017 performance year, the ACO reported $3.7 million shared savings and distributed 65 percent to ACO participants.

Capital Health Accountable Care Organization (Trenton, N.J.). Capital Health Accountable Care Organization serves patients in New Jersey and Pennsylvania. The Track 1 MSSP ACO launched in January 2015 and began its second agreement period in January 2018. During the 2017 performance year, the ACO reported $1.1 million in shared savings and distributed 35 percent to participants, including Pennington, N.J.-based Capital Health System.

CaroMont ACO (Gastonia, N.C.). CaroMont ACO launched in January 2014 in the Medicare Shared Savings Program. The ACO's 600-plus providers include CaroMont Medical Group, Gaston Memorial Hospital and Hospice of Gaston County, all in Gastonia, N.C. The organization reported $5.1 million in shared savings for the 2016 performance year. It entered a second agreement period for Track 1 in 2017.

Centra Alliance (Lynchburg, Va.). Centra Alliance has 14 participants, including Centra Health, a health system that serves more than 500,000 people in central and southern Virginia. The health system has about 400 employed care providers and physicians. The physician-led ACO includes both employed and independent physicians who aim to improve the quality of care in the Lynchburg, Va., area.

Carolinas HealthCare System ACO (Charlotte, N.C.). Carolinas HealthCare System ACO has 68 participants, including several hospitals, medical groups and urgent care centers. The ACO is affiliated with Charlotte-based Atrium Health and became a Track 1 MSSP ACO in January 2018. Atrium Health includes more than 40 hospitals and 900 care locations.

CHESS Value (High Point, N.C.). CHESS is a Track 1 MSSP ACO that launched in January 2017, serving patients in North Carolina and Virginia. High Point-based Cornerstone Health Enablement Strategic Solutions, a physician-led company owned by North Carolina Baptist Hospital in Winston-Salem, N.C., Cornerstone Health Care in High Point, and Laboratory Corporation of America in Burlington, N.C., administers the ACO. Two hospitals and several physician groups are in the ACO.

Christus Health Quality Care Alliance (Irving, Texas). Christus Health Quality Care Alliance is a Track 1 MSSP ACO serving patients in Texas, Louisiana and New Mexico. It launched in January 2016 and is in its first agreement period. The ACO is affiliated with Christus Southeast Texas Health System, a nonprofit system with three inpatient hospitals and 27 other points of care.

Cleveland Clinic Medicare ACO (Independence, Ohio). Cleveland Clinic Medicare ACO covers members in Ohio and Florida, where Cleveland Clinic has major campuses. The Track 1+ MSSP ACO launched in 2015 and began its second agreement period in January 2018. During the 2016 performance year, Cleveland Clinic Medicare ACO reported nearly $20 million in shared savings and distributed 90 percent to participants. The ACO is affiliated with Cleveland Clinic, an 18-hospital health system with more than 210 outpatient locations.

Coastal Plains Network (Greenville, N.C.). Launched in January 2015, Coastal Plains Network is a Track 1 MSSP ACO that serves patients in North Carolina. It began its second agreement in 2018. During the 2017 performance year, the ACO reported nearly $3.9 million in shared savings and distributed 60 percent to participants, which include Greenville, N.C.-based Vidant Medical Group.

ColigoCare (Paramus, N.J.). Although based in New Jersey, ColigoCare serves patients in New York as well. The Track 1 MSSP ACO launched in January 2018 and includes both primary care and specialty physicians. The clinically integrated network is a physician-led organization that aims to provide value-based care.

Collaborative Care of Florida (Orlando). Collaborative Care of Florida is a Track 1 MSSP ACO. It launched in January 2013 and entered into its second agreement in January 2016. The organization is affiliated with Orlando (Fla.) Health, which has seven acute care hospitals and two affiliated joint venture hospitals. The system comprises more than 20,000 team members and 3,000-plus physicians on staff. The ACO has consistently earned above 90 percent quality rating.

Deaconess Care Integration (Evansville, Ind.). Deaconess Care Integration participates in the Next Generation ACO Model, aiming to reduce expenditures while improving outcomes for Medicare fee-for-service beneficiaries. During the 2017 performance year, its second year in the Next Generation model, the ACO reported $16.1 million in shared savings. Evansville-based Deaconess Health System, which includes more than 30 care sites and serves patients in Indiana, Illinois and Kentucky, formed the ACO.

Delaware Valley ACO (Radnor, Pa.). Delaware Valley ACO is a Track 1 MSSP organization that launched in January 2014 and began its second agreement period in January 2017. It serves patients in Pennsylvania and New Jersey. The ACO has several participants, including Philadelphia-based Thomas Jefferson University Hospitals and Main Line HealthCare in Newtown Square, Pa.

DMH Health Network (Decatur, Ill.). DMH Health Network launched in January 2018 and is a Track 1 MSSP ACO. The ACO is affiliated with Decatur Memorial Hospital, a 300-bed nonprofit hospital with more than 30 satellite offices in central Illinois. The ACO is in its first reporting period.

Einstein Care Partners (Philadelphia). Formed in June 2017, Einstein Care Partners is a Track 1 MSSP ACO serving patients in Philadelphia. The ACO is comprised of primary care practices of Philadelphia-based Einstein Community Health Associates, Einstein Physicians Philadelphia and Einstein Physicians Montgomery in East Norriton, Pa. The primary care physicians care for around 12,000 beneficiaries through the ACO and serve 60,000 beneficiaries in total under population health payment contracts.

Essentia Health (Duluth, Minn.). Essentia Health is a Track 3 MSSP ACO that serves patients in Minnesota, North Dakota and Wisconsin. It launched in July 2012 and entered into its second agreement period in January 2016. Essentia Health Virginia (Minn.), as well as several critical access hospitals and medical groups participate in the ACO, which reported $2.8 million in shared savings for the 2017 performance year. It distributed 40 percent of the savings to participants.

Franciscan ACO (Mishawaka, Ind.). A Medicare ACO, Franciscan ACO also has commercial partnerships with Anthem, Humana and United Healthcare Medicare Advantage plans. It serves patients in Indiana and Illinois. Launched in January 2015, the Track 1 MSSP ACO entered into its second agreement period in January 2018. The ACO is affiliated with Indianapolis-based Franciscan Health, a 12-hospital health system.

Frederick (Md.) Integrated Healthcare Network. Frederick Integrated Healthcare Network has around 330 private practice physicians that developed the organization with Frederick (Md.) Regional Health System. Launched in January 2015, the physician-led network is a Track 1 MSSP ACO. It entered into its second agreement period in January 2018. The ACO reported $5 million shared savings in the 2016 performance year and distributed 80 percent to participants.

Genesis HealthCare ACO (Kennett Square, Pa.). Genesis HealthCare ACO launched in January 2016. It is a Track 1 MSSP ACO that serves patients in 11 states on the East Coast and in the Midwest. The ACO is affiliated with Genesis, a network of post-acute care providers that includes 400 skilled nursing centers and senior living communities. It has 1,200 locations across the U.S.

Hackensack Alliance ACO (Edison, N.J.). Hackensack Alliance ACO is a Track 1 MSSP ACO that launched in April 2012. It's on its second agreement period and includes three hospitals: Hackensack (N.J.) University Medical Center, Westwood, N.J.-based HackensackUMC at Pascack Valley and HackensackUMC Mountainside in Glen Ridge, N.J. In the 2017 performance year, Hackensack Alliance ACO reported $18 million shared savings and distributed 69 percent to ACO participants. Practices involved may become a certified patient-centered medical home.

Hartford Healthcare Accountable Care Organization (Wethersfield, Conn.). Hartford Healthcare Accountable Care Organization launched in January 2013 as part of the Medicare Shared Savings Program and entered into its second agreement in January 2016. It is a Track 1 MSSP ACO and is affiliated with Hartford (Conn.) HealthCare, a four-hospital health system that has 19,861 employees.

Healthcare Solutions Network (Cincinnati). Healthcare Solutions Network is an integrated network formed in March 2014 joining Covington, Ky.-based St. Elizabeth Healthcare and TriHealth in Cincinnati. The ACO has more than 1,600 providers and 10 hospitals in the tri-state region. The two organizations also worked together on a Centers for Medicare and Medication Innovation Services pilot for comprehensive primary care innovation. In January 2017, the network became a Track 1 MSSP ACO.

Healthy Communities ACO (Suffern, N.Y.). Healthy Communities ACO serves patients in New York, Pennsylvania and New Jersey. The Track 1 MSSP ACO launched in January 2017 and allows for more coordinated care for its 11,000 Medicare fee-for-service beneficiaries. The ACO is affiliated with Suffern-based Bon Secours Charity Health System, which is a member of the Westchester Medical Center Health Network in Valhalla, N.Y.

Henry Ford ACO (Detroit). Henry Ford ACO launched in January 2016 and participates in the Next Generation ACO Model. The organization includes more than 1,200 physicians and six hospitals. The organization is affiliated with Henry Ford Health System, a Detroit-based system with more than 30,000 employees.

Heritage California ACO (Northridge, Calif.). Heritage California ACO participated in the Pioneer ACO model for Medicare and has continued to grow. The ACO is affiliated with Heritage Provider Network in Northridge, Calif., which includes 2,300 primary care physicians, 30,000 specialists and more than 100 hospitals. It delivers patient-centered healthcare to 700,000-plus individuals through its provider network.

Ingalls Care Network (Harvey, Ill.). Ingalls Care Network became a Track 1 MSSP ACO in January 2014. The organization entered into its second agreement in January 2017. During the 2016 performance year, the ACO reported $2.8 million in shared savings and distributed 33 percent to ACO participants. The ACO is affiliated with UChicago Medicine.

Integra Community Care Network (Providence, R.I.). Integra Community Care Network began participating in the Next Generation ACO model with 19,000 Medicare beneficiaries in January 2017. The ACO includes Providence-based Care New England Health System, South County Health System in South Kingstown, R.I., Rhode Island Primary Care Physician Corporation in Cranston, R.I., and affiliated physicians. The ACO has more than 120,000 covered lives. During the 2016 performance year, when the ACO participated in the MSSP, Integra realized $8.3 million in shared savings.

Johns Hopkins Medicine Alliance for Patients (Glen Burnie, Md.). Johns Hopkins Medicine Alliance for Patients became a Track 1 MSSP ACO in 2014 and entered into its second contract in January 2017. The organization serves patients in Maryland and the District of Columbia. Johns Hopkins Medicine Alliance includes around 3,000 affiliated medical providers that care for around 38,000 Medicare patients in its region. During the second year after joining the shared savings program, the ACO reported reduced hospital admission and emergency department visits by 2 percent.

Kentucky Good Help ACO (Ashland). Kentucky Good Help ACO launched in January 2017 and is a Track 1 MSSP ACO. The organization includes Ashland-based Bellefonte Physician Services, Morehead, Ky.-based St. Claire Medical Center and Bon Secours Our Lady of Bellefonte Hospital in Russell, Ky.

CHI St. Joseph Health Partners (Lexington, Ky.). CHI Saint Joseph Health Partners includes more than 100 provider groups and nearly 2,000 providers in Kentucky, Ohio and Indiana. The ACO manages around 100,000 lives, and it recently recertified as part of the MSSP in August. The ACO is affiliated with CHI Saint Joseph Health in Lexington, formerly KentuckyOne Health, which includes 135 locations.

Kootenai Accountable Care (Coeur d'Alene, Idaho). Kootenai Accountable Care became a Track 1 MSSP ACO in January 2018. The ACO is a clinically integrated network affiliated with Coeur d'Alene-based Kootenai Health, which includes a 330-bed community-owned hospital.

Lahey Clinical Performance ACO (Burlington, Mass.). Lahey Clinic Performance ACO became a Track 1 MSSP ACO in January 2013 and began its second agreement period in January 2016. The ACO is one of 18 participants in Quincy-based MassHealth's ACO program, which also began January 2018. MassHealth ACOs cover more than 900,000 members and include 4,500 primary care providers. In the 2017 performance year, the ACO had $4.6 million in shared savings and distributed 33 percent to participants.

LifeBridge Health ACO (Baltimore). LifeBridge Health ACO is a Track 1 MSSP that began in January 2015 and entered into its second reporting period three years later. During the 2017 performance year, the ACO reported $6.8 million in shared savings and distributed 50 percent to participants. The ACO includes more than 18,000 beneficiaries and 430 providers, with 130-plus being primary care physicians. The ACO covers around 20,000 lives and achieved a 92.5 percent quality rating in 2017.

MaineHealth Accountable Care Organization (Portland, Maine). MaineHealth ACO is a 10-hospital network that includes more than 1,600 healthcare providers. It cares for around 200,000 patients annually in Maine and northern New Hampshire. MaineHealth ACO became a Track 1 MSSP in July 2012 and began its second reporting period in January 2016. Earlier this year, the ACO reported it would continue participating in the MSSP program for the next five years, beginning to assume downside risk in 2021.

Mary Washington Health Alliance (Fredericksburg, Va.). Mary Washington Health Alliance is a clinically integrated network founded in 2013. The ACO has 437 participants that cover around 80,000 lives. During the 2017 MSSP performance year, the ACO achieved $11.9 million in savings. For the first three years it participated in the CMS Bundled Payment for Care Improvement program, it achieved $12.6 million in savings. The ACO now participates in the Next Generation ACO model and is active in the BPCI Track 2 for all 48 episodes of care.

Maury Regional Health Network (Columbia, Tenn.). Maury Regional Health Network is a Track 1 MSSP ACO serving patients in Tennessee. The ACO entered into the MSSP program in January 2017 and is in its first reporting period. The ACO is affiliated with Columbia-based Maury Regional Health, which cares for around 250,000 people in southern Middle Tennessee.

MedStar Accountable Care (Baltimore). This Baltimore-based accountable care network is a Track 3 MSSP that entered the program in January 2016. It serves Columbia, Md.-based MedStar patients in the Maryland and Washington, D.C., area. The ACO also participates in the Maryland Primary Care Program as a care transformation organization, sharing innovation in primary care services statewide.

Memorial Hermann ACO (Houston). Memorial Hermann Accountable Care Organization includes Houston-based Memorial Hermann's 12 hospitals, as well as other institutes and outpatient facilities. It also unites independent and employed physicians across multiple specialties to improve quality of care. Memorial Hermann ACO became a Track 1 MSSP ACO in July 2012 and is in its second reporting period. During the 2017 performance year, Memorial Hermann ACO reported $14.5 million in shared savings and distributed 65 percent to ACO participants.

MercyOne ACO (Des Moines, Iowa). MercyOne ACO includes around 500 practice sites and 2,300 physicians. In five years, the organization helped save more than $65 million in healthcare costs with a laser focus on providing quality and cost-efficient care. MercyOne provides managed services for four ACOs.

Mercy Health Select (Cincinnati). Mercy Health Select serves patients in Ohio and Kentucky. The Track 3 MSSP joined the program in July 2012 and entered its second reporting period in January 2016. During the 2017 performance year, the organization reported $2.3 million in shared savings. The clinically integrated network includes hospitals and physicians across Cincinnati-based Mercy Health system, which has more than 500 locations.

Mercyhealth ACO (Janesville, Wis.). Mercyhealth ACO covers around 20,300 lives and, in the 2017 and 2018 fiscal years, it reached 60 percent of beneficiaries with an annual wellness visit. The ACO achieved $7.2 million in savings for the 2017 performance year and includes more than 80 primary and specialty locations. It has an integrated standardized medical record system across all locations, and Mercyhealth serves around 2.4 million patients in Wisconsin and Illinois.

Methodist Alliance for Patients and Physicians (Dallas). Methodist Alliance for Patients and Physicians is a Track 1 MSSP ACO that joined the program in July 2012. It began its second reporting period in January 2016. More than 600 physicians partner with the ACO, which has Dallas-based Methodist Health System as its sole member. In the 2017 performance year, the ACO reported $6.8 million in shared savings and distributed it evenly between investments in infrastructure, redesigning care processes and distributions to ACO participants.

Mid-Atlantic Collaborative Care (Houston). Mid-Atlantic Collaborative Care became a Track 1 MSSP in January 2017. It serves patients across Maryland, Virginia, West Virginia, Pennsylvania and Washington, D.C. During the 2017 performance year, the ACO reported $2.4 million in shared savings and distributed 12.2 percent to ACO participants. The ACO serves more than 16,000 Medicare beneficiaries.

Millennium ACO (Fort Myers, Fla.). Millennium ACO is a Track 3 MSSP that joined the program in January 2013. The Florida-based organization entered its second contract period in January 2016. The organization focuses on primary care providers. Millennium ACO reported $29.7 million in shared savings for the 2017 performance year and distributed half to ACO participants.

Montefiore ACO (New York City). Montefiore ACO launched in 2012 and was accepted to participate in the Next Generation ACO model in 2017. The health system includes 11 hospitals in New York, as well as primary care sites. The ACO is affiliated with employed and community-based physicians in the Bronx, Westchester, Brooklyn, Staten Island and Hudson Valley. During its second performance year, the ACO reported $11.7 million in shared savings and distributed 18 percent to providers; the remaining was reinvested in the ACO's infrastructure.

New York Medical Partners (Oceanside, N.Y.). New York Medical Partners ACO is affiliated with New York City-based Mount Sinai Health System. The organization is an MSSP ACO. The health system has more than 400 locations across New York, as well as 38 institutes, and employs more than 42,000 individuals, including 7,400 primary care physicians and specialists.

National Rural Accountable Care Consortium (Beaverton, Ore.). National Rural ACO was developed by thought leaders in rural healthcare who also founded NRACO Services Corp., which supports rural Medicare ACOs. The organization provides data, analytics and patient satisfaction surveys, as well as evidence-based medicine programs. The organization included more than 6,000 providers in 159 rural health systems by 2015. The organization also received $46 million in funding to support the program, which includes 92 critical access hospitals, 168 rural health clinics and more than 500,000 Medicare patients.

NewHealth Collaborative (Akron, Ohio). NewHealth Collaborative is the ACO of Summa Health System and includes more than 500 providers spanning six hospitals. It became a Track 1 MSSP ACO in 2012 and serves patients in Ohio. The ACO entered its second agreement period in January 2016. NewHealth Collaborative covers around 72,000 lives and reports $600 million-plus in total Medicare spend.

NewYork Quality Care (New York City). NewYork Quality Care became a Track 1 MSSP ACO in January 2015 and began its second reporting period in January 2018. The ACO affiliates include New York City-based NewYork-Presbyterian, a health system with more than 6,500 affiliated physicians and 20,000 employees, as well as Columbia University College of Physicians and Surgeons and Weill Cornell Medicine, both in New York City.

Northwell Health ACO (New Hyde Park, N.Y.). Launched in January 2016, Northwell Health ACO is a Track 1 MSSP ACO serving patients in New York and Connecticut. The ACO is governed by independent and New Hyde Park, N.Y.-based Northwell Health-employed physicians, as well as a Medicare fee-for-service beneficiary. Northwell includes 3,000 physicians, 750 hospitals and care centers, and more than 70,000 employees.

NYC Health + Hospitals (New York City). NYC Health + Hospitals is an 11-hospital health system that serves more than 1 million patients in New York City. The system has more than 70 locations across New York City's five boroughs. The system's ACO is a member of the MSSP, and it reported $2.1 million in shared savings for the 2017 performance year. The ACO distributed 43 percent of savings to participants and reinvested the rest in redesigned care processes and resources for the ACO.

OhioHealth Venture (Columbus). OhioHealth Venture is a Track 1 MSSP ACO that launched in January 2018. The ACO is still in its first reporting period and is affiliated with Columbus-based OhioHealth, a 12-hospital health system with more than 200 ambulatory and home health sites. It includes 29,000 associates, physicians and volunteers.

OneCare Vermont (Colchester). OneCare Vermont is an ACO that works with commercial insurers, Medicare, Vermont Medicaid and self-funded insurance programs. It includes a network of 2,798 primary and specialist physicians and 13 hospitals. The ACO supports around 172,000 individuals across Vermont. It was established as a joint venture between the Burlington-based University of Vermont Medical Center and Dartmouth-Hitchcock Medical Center in Lebanon, N.H.

Optimus Healthcare Partners (Summit, N.J.). Optimus Healthcare Partners is a Track 1 MSSP ACO in its second reporting period, which began in January 2016. The ACO reported $4.3 million in shared savings for the 2017 performance year and distributed 70 percent to ACO participants. The ACO includes commercial and Medicare fee-for-service beneficiaries. Optimus has around 1,034 participants and covers about 120,000 lives. It achieved a 90 percent or greater quality rating for five consecutive years and earned $50 million in savings for CMS since its inception.

Palm Beach Accountable Care Organization (Palm Springs, Fla.). Launched in July 2012, Palm Beach Accountable Care Organization entered its second agreement period in January 2016. The Track 1 MSSP ACO reported $28.5 million in shared savings during the 2017 performance year and distributed 85 percent to the organization's participants.

Park Nicollet Health Services (St. Louis Park, Minn.). Park Nicollet has been a Next Generation ACO model participant since 2016. The ACO reported shared savings of $3.5 million in the 2017 performance year and reinvested all the savings into infrastructure to support patient care. That year, the ACO cared for around 12,600 patients and reported a 93 percent quality score. It also was able to lower costs for Medicare by $4.5 million.

Partners HealthCare (Boston). Partners HealthCare is a nonprofit system founded by Brigham and Women's Hospital and Massachusetts General Hospital, both in Boston. Many of its hospitals are teaching affiliates of Boston-based Harvard Medical School. The health system has had ACO contracts with all major insurers, including CMS, since 2012. The ACO had been part of the Next Generation ACO model but transitioned to the MSSP in the summer of 2019. During five years as a pioneer ACO, the organization reported saving $38 million with a 94.5 percent quality score.

Partners for Kids (Columbus, Ohio). As a partnership between more than 1,000 physicians and Columbus-based Nationwide Children's Hospital, Partners for Kids serves around 330,000 children in central and southeastern Ohio. It was founded in 1994 and provides care coordinators to guide patients through their healthcare journeys. Based on 2014 data, patients who enrolled in the ACO for at least 120 days had fewer inpatient admissions and emergency department visits than patients who were not enrolled.

Physician Partners of Western PA (Pittsburgh). Physician Partners of Western PA became a Track 1 MSSP ACO in January 2018. The physician-led clinically integrated network includes both independent and employed physicians, and it covers patients in 14 counties in western Pennsylvania. The ACO is sponsored by Pittsburgh-based Allegheny Health Network, an eight-hospital health system that includes 2,800 employed or aligned physicians and 18,000 employees.

Physician Organization of Michigan ACO (Ann Arbor). Physician Organization of Michigan ACO reported $20.7 million in shared savings for the 2017 performance year and distributed 49 percent to ACO participants. The ACO is affiliated with Ann Arbor-based Michigan Medicine, Wexford/Crawford Physician Hospital Organization, MidMichigan Collaborative Care Organization and Oakland Southfield (Mich.) Physicians.

Physicians of Southwest Washington (Olympia). Physicians of Southwest Washington is an independent physician association formed in 1995 by primary care and specialty physicians. The organization contracts with Medicare Advantage health plans and includes more than 500 healthcare providers that serve around 5,000 patients. In 2016, Physicians of Southwest Washington launched NW Momentum Health Partners ACO to participate in the Next Generation ACO model and coordinate care for around 9,000 Medicare beneficiaries. For the 2017 performance year, the ACO reduced spending by 8.2 percent and achieved $3.3 million in shared savings.

Piedmont Clinic ACO (Atlanta). Piedmont Clinic ACO is a Track 1 MSSP ACO that launched in January 2017 and serves patients in Georgia and Alabama. The clinically integrated network is affiliated with Atlanta-based Piedmont Clinic, which is composed of around 2,000 physicians, including independent practitioners or those employed by Piedmont Physician Enterprise in Atlanta.

Pioneer Valley Accountable Care (Springfield, Mass.). Pioneer Valley Accountable Care participates in the Next Generation ACO model with Springfield-based Baycare Health Partners as its sole member. Baycare is a physician hospital organization with four hospitals and over 200 medical practices. The system has around 1,300 physicians who collaborate to improve patient care. During the 2017 performance year, the ACO reported $14.3 million in shared savings and distributed 83 percent to participants and preferred providers.

Princeton HealthCare Partners (Plainsboro, N.J.). Princeton HealthCare Partners is a Track 1 MSSP ACO serving patients in New Jersey and Pennsylvania. It joined the MSSP in January 2016 and is in its first reporting period. The ACO includes more than 90 primary care physicians and serves 7,800-plus Medicare fee-for-service beneficiaries.

ProHEALTH Accountable Care Medical Group (Lake Success, N.Y.). ProHEALTH Accountable Care Medical Group became a Track 1 MSSP ACO in July 2012 and began its second reporting period in January 2016. The ACO is affiliated with ProHEALTH in Lake Success, part of Chicago-based OptumCare, which includes both primary care physicians and specialists.

ProHealth Solutions (Waukesha, Wis.). ProHealth Solutions participates in the Next Generation ACO model. During its first performance year, the ACO reported $5.6 million in shared savings. Waukesha-based ProHealth Care, which includes an acute care hospital, a major tertiary care center, a rehabilitation hospital and a medical group with 12 clinics, has participants in the ACO. The health system employs around 4,700 staff members and hundreds of physicians, along with other healthcare professionals.

ProMedica Health Network (Toledo, Ohio). ProMedica Health Network is focused on providing high-quality, cost-effective care. The physician-led ACO pinpoints contracting opportunities for providers and distributes payer contract offers. It became a Track 1 MSSP ACO in January 2016 and serves patients in Ohio and Michigan. It is affiliated with ProMedica, a 13-hospital health system in Toledo that has almost 56,000 employees and more than 2,100 physicians.

Queen's Clinically Integrated Physician Network (Honolulu). Queen's CIPN launched in 2014 as a collaboration between physicians, physician organizations and The Queen's Health Systems in Honolulu. The organization includes more than 800 providers on five Hawaiian islands. Queen's Health Systems has more than 70 locations throughout the islands and 1,500-plus physicians statewide, as well as more than 7,450 employees.

Richmond (Va.) Good Help ACO. Richmond Good Help ACO launched in January 2017 and is a Track 1 MSSP ACO. During the 2017 performance year, Richmond Good Help reported $3.7 million in shared savings and distributed 75 percent to ACO participants. Four hospitals from the Cincinnati-based Bon Secours Mercy Health System, as well as an orthopedic group and multiple physician groups, participate in the ACO.

Rio Grande Valley ACO Health Providers (Donna, Texas). RGV ACO Health Partners has locations in four counties in Texas and one in New Jersey. During the 2017 performance year, the ACO reported $9.4 million in shared savings and distributed 72 percent to participants. The ACO is a recognized patient-centered medical home with 15 certified PCMH clinics.

Rio Grande Valley Health Alliance (McAllen, Texas). Rio Grande Valley Health Alliance comprises 22 primary care physicians aiming to provide high-quality care. Launched in January 2013, Rio Grande Valley Health Alliance entered into its second agreement period three years later. The Texas-based organization, a Track 3 MSSP ACO, reported $6.8 million in shared savings during the 2017 performance year and distributed 80 percent to ACO participants.

Riverside Health Source (Newport News, Va.). Riverside Health Source is a Track 1 MSSP ACO that joined the program in January 2018. The ACO is affiliated with Riverside Health System in Newport News, a nonprofit integrated health system that serves more than 2 million patients each year. The system also includes three surgery centers, retirement communities, home care, and a wellness and fitness center.

Rochester (N.Y.) Regional ACO. This Track 1 MSSP ACO launched in January 2017 and is in its first agreement period. The ACO is a partnership between hospitals and healthcare providers. It is affiliated with the Greater Rochester Independent Practice Association, which includes more than 1,300 physicians and affiliated hospitals.

Robert Wood Johnson Partners (New Brunswick, N.J.). The Robert Wood Johnson Partners ACO aims to improve quality care. It is an integrated delivery system that includes more than 700 physicians from New Brunswick-based Rutgers University and Princeton, N.J.-based Robert Wood Johnson Health System. During the 2015 reporting year, the ACO had 37,497 beneficiaries and six hospitals participating.

Roper St. Francis Health Alliance (Charleston, S.C.). Roper St. Francis Health Alliance is a Track 1 MSSP ACO that joined the program in January 2017. The ACO is affiliated with Charleston-based Roper St. Francis and includes both independent and employed physicians. The clinically integrated network is physician-led and aims to focus on wellness and individual care plans for patients.

Rush Health ACO (Chicago). Rush Health ACO became a Track 1 MSSP ACO in January 2018. The organization is an affiliate of Chicago-based Rush Health, a four-hospital health system that has 1,100 physician members and 300 non-physician network clinicians.

SERPA-ACO (Crete, Neb.). SERPA-ACO is a Nebraska-based Track 1 MSSP ACO that began in January 2013 and entered its second agreement period three years later. Sixteen clinics in Nebraska make up SERPA-ACO. The ACO relies on the patient-centered medical home model.

Signature Partners (Falls Church, Va.). Signature Partners is a Track 1 MSSP ACO that entered into its second agreement period in January 2018, three years after it launched. The ACO is a clinically integrated network of more than 1,700 physicians with a mission embodying the goals of the Triple Aim.

Southcoast Accountable Care Organization (New Bedford, Mass.). Southcoast Accountable Care Organization serves patients in Massachusetts and Rhode Island. The Track 1 MSSP ACO began in January 2013 and entered into its second reporting period in January 2016. The ACO has 20 participants, including Fall River, Mass.-based Southcoast Hospitals Group. The organization is affiliated with Southcoast Health in Fall River, which has 450 providers and more than 55 service locations.

Southeastern Health Partners Medicare ACO (Greenville, S.C.). Southeastern Health Partners became a Track 1 MSSP ACO in January 2017 and serves patients in both North and South Carolina. The ACO has nine participants, including Anderson, S.C.-based AnMed Health, Greenville-based St. Francis Physician Services and Spartanburg (S.C.) Medical Center.

Southwestern Health Resources (Dallas). Southwestern Health Resources comprises 29 hospitals and 4,000-plus physicians that aim to improve quality of care in North Texas. The ACO has around 600,000 patients and more than 500 points of access across its network. For the 2017 performance year, CMS reported Southwestern Health Resources saved $30 million and had a 100 percent quality score. It was also approved to participate in the Next Generation ACO model. Both Arlington-based Texas Health Resources and Dallas-based UT Southwestern Medical Center are affiliated with the ACO.

St. Joseph's/Candler Advocate Health Network (Savannah, Ga.). This Track 1 MSSP ACO joined the program in January 2017 and serves patients in Georgia and South Carolina. It is a physician-led organization with Savannah-based St. Joseph's/Candler Health System as its only member. The health system includes two hospitals and 714 patient beds.

St. Luke's ACO (Chesterfield, Mo.). St. Luke's ACO became a Track 1 MSSP ACO in January 2018 and serves patients in Missouri and Illinois. The ACO is affiliated with St. Luke's Hospital in Chesterfield, a two-hospital health system that includes a total of 636 beds and more than 30 outpatient locations.

St. Luke's Medicare ACO (Bethlehem, Pa.). St. Luke's Medicare ACO is a Track 1 MSSP ACO that launched in January 2018. The organization is affiliated with St. Luke's Health Network in Bethlehem, Pa., a nonprofit health system with more than 300 sites in Pennsylvania and New Jersey. The network is also affiliated with 1,156 physicians and advanced practice providers.

Steward Health Choice Arizona (Phoenix). Steward Health Choice Arizona consists of four Steward Health Care Network hospitals in Phoenix. Altogether, Dallas-based Steward Health Care has 37 community hospitals in nine states as well as 42,000 employees. The health system serves more than 800 communities and reports 12 million patient encounters per year.

Summa Accountable Care Organization (Akron, Ohio). Summa ACO has 27 participants, including individual physicians and practices. The organization reported more than $13 million in total shared savings for its first three performance years. It is affiliated with Akron-based Summa Health, a hospital network that provides care to more than 1 million individuals across five markets.

ThedaCare ACO (Appleton, Wis.). ThedaCare ACO participates in the Next Generation ACO model for more coordinated care. Appleton-based ThedaCare, which is affiliated with the ACO, is a seven-hospital health system that has 35 clinics and serves around 235,000 patients in Wisconsin.

Trinity Health ACO (Livonia, Mich.). Trinity Health ACO serves patients in Michigan, Illinois, New Jersey and Ohio. It participates in the Next Generation ACO model. During the 2017 performance year, the ACO achieved $8.4 million in shared savings and a 96 percent quality performance score. The ACO has around 85,300 beneficiaries and achieved 29 percent higher year-over-year savings in the most recent performance year.

UMC Accountable Care (Lubbock, Texas). UMC Accountable Care launched in January 2018 and is a Track 1 MSSP ACO serving patients in Texas and New Mexico. The ACO includes two physician groups — UMC Physicians and Texas Tech University Health Science Center Physicians, both based in Lubbock — and has a total of 325 physicians. University Medical Center in Lubbock is also part of the organization.

UnityPoint Accountable Care (West Des Moines, Iowa). UnityPoint Accountable Care participates in the Medicare Next Generation ACO model. It includes a group of physicians and hospitals working together to improve quality of care while lowering costs. During the 2017 performance year, the ACO reported $11.5 million in shared savings and distributed 100 percent to participants. West Des Moines-based UnityPoint Health, which is affiliated with the ACO, includes around 30,000 team members.

University Health ACO (Knoxville, Tenn.). University Health ACO became a Track 1 MSSP ACO in January 2018 and is still in its first reporting period. The ACO is affiliated with the University of Tennessee Medical Center, a 640-bed hospital in Knoxville that serves as a major regional referral center.

University Hospitals Coordinated Care Organization (Shaker Heights, Ohio). University Hospitals Coordinated Care Organization has pediatric, employee and Medicare ACOs. Starting in 2010, UHACO has served around 25,000 employees and dependents, aiming to improve health and wellness. Its pediatric ACO, affiliated with Cleveland-based UH Rainbow Babies & Children's Hospital, aims to serve 200,000 children. UH's Medicare ACO comprises around 42,000 Medicare beneficiaries.

UW Health ACO (Madison, Wis.). UW Health and UnityPoint Health-Meriter, both based in Madison, as well as Madison-based University of Wisconsin School of Medicine and Public Health, teamed up to form the UW Health ACO. The organization participates in the Next Generation ACO model. UW Health is a six-hospital health system that includes 80 outpatient sites and more than 1,500 physicians who serve 600,000 patients annually.

WakeMed Key Community Care (Raleigh, N.C.). WakeMed Key Community Care is a Track 1 MSSP ACO that joined the program in January 2017. Raleigh-based WakeMed Health & Hospitals and Cary, N.C.-based Key Physicians formed the ACO to improve the quality of care. WakeMed is a nonprofit, 884-bed health system that includes 1,200 affiliated physicians. Key Physicians has around 200 primary care physicians in its medical home network, and WakeMed Physician Practice has more than 240 physicians in its network

WellStar Clinical Partners (Marietta, Ga.). WellStar Clinical Partners is a collaboration between around 1,800 physicians. It was established in 2015 to improve healthcare, patient experiences, service and value across its region. The organization is affiliated with Marietta-based WellStar Health System, an 11-hospital network that includes 225 medical office locations and around 20,000 team members.

West Florida ACO (Trinity, Fla.). West Florida ACO was one of the first 27 ACOs accepted by Medicare in April 2012 for the shared savings program. It is now a Track 3 MSSP ACO that entered its second agreement period in January 2016. In the 2017 performance year, the ACO reported $8.8 million in shared savings and distributed 62 percent to ACO participants.

White River Health System Clinically Integrated Network (Batesville, Ark.). White River Health System Clinically Integrated Network became a Track 1 MSSP ACO in January 2018. The ACO has seven participants, including Batesville-based White River Health System, a two-hospital organization with outpatient centers and clinics across Arkansas.
